Must bring Driver's License / Gov't ID and Insurance Card to testing site.

When you arrive at the facility for your test, please be prepared to display your PRIVIT Pass profile ID QR code to lab personnel from your mobile device.
If you have not already done so, please be sure to complete all of the required forms within this portal. You must upload a copy of your insurance card (if applicable) and your US or State Government ID or Driver's license using the upload documents button on your profile and have those items with you upon arrival at the testing site.
If you have not completed these steps it will delay your test processing or potentially provent your sample from being processed. When complete, your profile will display a green bar along the top reading SUBMISSION COMPLETE.
